The Vessel HMS Affray (P421) was a British A-class submarine of the Royal Navy, commissioned at the Cammell Laird yard at Birkenhead on 25 November 1945. She was 87 metres long, 1,120 tons surfaced displacement, 1,620 tons submerged, armed with ten 21-inch torpedo tubes (six forward, four aft) and a single 4-inch gun. Her designed operational depth was 150 metres; her test depth was 180 metres. The A-class was the Royal Navy's last generation of conventional diesel-electric submarine built during the Second World War, commissioned between 1945 and 1948. Sixteen A-class submarines were built at various British yards; Affray was the eighth of the class. The Disaster At 12:00 on 17 April 1951 HMS Affray did not surface as scheduled. The Portsmouth submarine command initiated the standard overdue-submarine alert at 12:30 on 17 April. The Royal Navy's subsequent search operation was one of the most extensive peacetime submarine searches ever mounted. Over 50 surface ships, 4 submarines, and continuous RAF Coastal Command maritime patrol aircraft operations searched the Affray's expected operating area for 60 days between 17 April and 14 June 1951. HMS Affray was located on 14 June 1951 by the submarine search vessel HMS Loch Fada using an experimental Type 193 sonar that the Royal Navy had been developing specifically for submarine-location operations. Her wreck was found in 86 metres of water at approximately 50°17′N 2°17′W, on the continental shelf south of Dorset, on a relatively smooth sandy bottom. Subsequent investigation of the wreck, conducted by the recovery vessel HMS Reclaim using both surface-supplied diving and the newly-developed tethered remotely-operated underwater vehicle, identified the immediate cause of Affray's loss. Her snorkel mast, a 6-metre vertical pipe that allowed air intake when she operated at periscope depth, had failed structurally at the base. She was 118 metres long, 3,898 gross tons, and powered by twin steam turbines producing approximately 5,400 horsepower. Her specific design was a combined passenger-and-railway-car ferry intended for the Seikan Strait route between Aomori (on the northern tip of Honshu island) and Hakodate (on the southern tip of Hokkaido island). The Seikan Strait ferry service was a critical piece of Japanese national transport infrastructure. The strait, approximately 20 kilometres wide, had no bridge or tunnel connection until the Seikan Tunnel opened in 1988, meaning that all passenger and rail traffic between Hokkaido and Honshu had to transit by ferry. The Vessel The SS Andrea Doria was the postwar flagship of Italy's reconstituted merchant marine, built at the Ansaldo yard in Genoa and entered into Italian Line service on 14 January 1953. She was 213 metres long, 29,083 tons gross, named for the sixteenth-century Genoese admiral and statesman whose family had financed the yard that built her. At the moment of her launch she was the most technologically ambitious liner Italy had ever put to sea. She was designed for the New York-Genoa run, eleven classes between first and tourist, capable of 23 knots on two shafts, with a length-to-beam ratio and a hull form that let her press her speed through the North Atlantic in most weather. Her safety provisions exceeded the SOLAS requirements of the 1948 convention: she carried lifeboats for 2,000 when her full capacity was 1,241 passengers and 563 crew, and her eleven watertight compartments were individually calculated to sustain single-compartment flooding at any angle. She was also the most heavily styled liner of her generation. The Italian Line commissioned murals, tapestries, mosaics, and sculpture from Salvatore Fiume, Emanuele Luzzati, and Giovanni Ponti; her first-class Winter Garden carried a bronze statue of Admiral Andrea Doria cast for the ship. The rescue that followed was the largest civilian maritime rescue operation in history. The French liner Île de France, the American freighter Cape Ann, the U.S. destroyer Edward H. Allen, and the Stockholm herself worked the scene through the night. 1,660 people were taken off across eleven hours. The Andrea Doria sank at 10:09 on the morning of 26 July, ten hours and fifty-nine minutes after the collision, with her stern rising and her bow still on even keel against the fog. The Vessel The Pamir was a German four-masted steel-hulled sailing ship (barque rig), built at the Blohm & Voss yard at Hamburg between 1905 and 1905 and launched on 29 July 1905. She was 114 metres long, 3,020 gross tons, and had a carrying capacity of approximately 4,500 tonnes of cargo. She was among the final generation of commercial sailing ships built for deep-water cargo trade: the "Flying P-Liners" of the F. Laeisz shipping company, which operated a fleet of large sailing ships for the Chilean nitrate and Australian grain trades into the early twentieth century. Her specific operational history was notable. She had served through two world wars in various commercial roles; she had been interned by the Allies during both world wars and had subsequently returned to commercial service. In 1951, she had been purchased by a West German consortium specifically for training purposes: she was refitted for dual use as a commercial cargo carrier and as a sail-training vessel for the West German merchant marine. Wind velocity rose rapidly from force 7 to force 10 (approximately 90-100 kilometres per hour); sea state increased to approximately 7-8 metre waves; visibility reduced to less than 500 metres. The critical failure mechanism was the cargo shifting that the specific SOLAS-violation free-surface non-correction had not prevented. As Pamir began rolling heavily in the hurricane conditions, the barley cargo (approximately 3,790 tonnes) shifted to the leeward side of the cargo holds. The cumulative cargo shift produced a permanent lee-side list of approximately 15 degrees within approximately two hours of the hurricane's onset. The Vessel USS Stickleback (SS-415) was an American Tench-class submarine of the United States Navy, built at the Mare Island Naval Shipyard at Vallejo, California between 1944 and 1945 and commissioned on 29 March 1945. She was 95 metres long, 1,570 tons surfaced displacement (2,415 tons submerged), and armed with ten 21-inch torpedo tubes plus one 5-inch deck gun. The Tench-class was the final development of the American Balao-class fleet submarine design that had dominated the Pacific War. Her wartime service was minimal: commissioned in the final weeks of the Pacific War, she had conducted one war patrol (August 1945) before the Japanese surrender on 2 September 1945. Her subsequent postwar service through 1952 was as a Pacific Fleet training and experimental submarine. She had been selected in 1952 for GUPPY conversion (Greater Underwater Propulsion Power Program), the US Navy's modernisation programme for Second World War-era submarines. The investigation produced the SUBSAFE quality-control program, which has overseen the construction of every U.S. Navy submarine since. The Vessel USS Thresher (SSN-593) was the lead ship of the Permit-class nuclear attack submarines of the United States Navy, the design that represented the second generation of American nuclear attack boats after the Skipjack class. At 730 metres the seawater pressure is roughly 74 atmospheres; the collapse of a submarine hull under such pressure is complete in approximately 100 milliseconds. The crew, including the 17 civilian observers, died without any meaningful interval of awareness. The Navy's investigation identified the proximate cause as the failure of a silver-brazed joint in the saltwater piping system at or near test depth. Salt water entered an engine-room space, shorted an electrical panel, and initiated an emergency reactor scram. The reactor scram removed main propulsion. The emergency main ballast blow failed to complete because moisture had clogged the air-drying strainers of the emergency blow system. Thresher sank from test depth to implosion without the means to arrest her descent. The subsequent Naval Court of Inquiry and the follow-on DSRV submarine-rescue programme produced the most consequential reform in American submarine construction history. The SUBSAFE programme, established 20 December 1963 by direction of the Chief of Naval Operations, mandated extensive redesign of every component of every American submarine whose failure could result in flooding at depth. The Disaster At 14:20 on 25 May 1982 Argentine A-4 Skyhawk attack aircraft launched from the Argentine airfield at Río Gallegos were detected on HMS Coventry's radar at a range of 130 kilometres. Captain Hart-Dyke engaged with his Sea Dart missiles at 14:35; one Sea Dart shot down an Argentine Skyhawk at a range of approximately 40 kilometres. A second Argentine strike of four Skyhawks reached the Coventry-Broadsword position at 14:50. The second strike was the critical one. The four Argentine Skyhawks, carrying four 500-kilogram bombs between them, approached the British ships at very low altitude to avoid the Sea Dart radar. The low-altitude approach placed the aircraft below the minimum engagement altitude for the Sea Dart system. Coventry's close-in defences (her 4.5-inch gun and her 20mm Oerlikon guns) and Broadsword's Sea Wolf point-defence system were engaged to intercept the low-altitude strike. Ballast water was moved between the pontoons through internal piping to maintain stability in sea conditions. The specific ballast-control system was operated from a ballast control room on the main deck. Her commanding officer (the "toolpusher") in February 1982 was Richard Brooks, 34, a Canadian offshore industry veteran. She was 126 metres long, 5,674 tons fully loaded, and was designed as a general-purpose amphibious logistics ship rather than as a combat vessel. The Vessel The ARA General Belgrano (C-4) was an Argentine Navy light cruiser, originally commissioned into the United States Navy as USS Phoenix (CL-46), a Brooklyn-class light cruiser, on 3 October 1938. She was 186 metres long, 10,800 tons standard displacement, and armed with fifteen 6-inch guns in five triple turrets. Her American service included the attack on Pearl Harbor on 7 December 1941 (she was at her mooring when the attack began but sustained no damage and sortied within hours), subsequent Pacific War combat operations, and decommissioning in 1946. The Argentine Navy acquired her from the United States in 1951 for approximately $7.8 million. She was recommissioned as ARA 17 de Octubre (commemorating the date of the 1945 Peronist mass demonstration that had returned Juan Perón to power) and served as the Argentine Navy's principal flagship through the 1950s. The critical attacks came from Argentine Skyhawks at approximately 17:00. Three Skyhawks approached Ardent from the west at very low altitude, each carrying a single 500-kilogram bomb. The Skyhawks released their bombs at ranges of 100-200 metres from Ardent's stern. The Vessel The MS Herald of Free Enterprise was a drive-through roll-on/roll-off ferry built at Bremer Vulkan Schiffbau in Bremen in 1980 for the British ferry operator Townsend Thoresen, from November 1986 European Ferries. She was 131 metres long, 7,951 gross tons, certified to carry 1,400 passengers, 350 cars, or a combination; her design speed was 22 knots on twin screws. She was the eighth "Spirit" class ship on the short-sea Dover Strait routes. The Spirit class was the first generation of the full-drive-through ferry, a design in which cars and lorries entered the ship at the bow and exited at the stern without turning. The architecture required a continuous vehicle deck the length of the ship, with bow and stern doors that opened as two pairs: outer doors swinging outward, and an inner door closing over the vehicle ramp itself. The inner doors were the ship's watertight seal. They had to be closed before the ship could move. The closing of the inner doors was the responsibility of the assistant bosun. This was a matter of settled practice rather than formal procedure; no written standing order specified who should confirm closure or how confirmation should be passed to the bridge. The company's operational manuals, the inquest later established, contained no procedural description of this step at all. The bridge crew were expected to see that the bow doors were closed by looking out of the bridge windows; they could not see the inner doors from the bridge. They could not confirm The Disaster The entrance to Zeebrugge harbour is a narrow channel with a sea wall; the design of the ferry terminal required ships to gather speed quickly to enter the navigation channel of the Scheldt approach. As Herald of Free Enterprise cleared the harbour mouth and accelerated, her bow wave rose until it began to pour into the open vehicle-deck mouth. The first water entered the vehicle deck at approximately 18:27. The free surface effect was immediate. Once water is on a ship's vehicle deck it destabilises the ship in a way that no compensating action can reverse once a critical angle has been reached: the water slides to whichever side the ship is already leaning, which compounds the lean, which increases the slide. Herald of Free Enterprise's vehicle deck was open plan, 115 metres long, without subdivision. The water that entered moved to port. The ship developed a list to port of 30 degrees within 90 seconds. At 18:28 she capsized onto her port side. Had she been in deeper water she would have sunk. Her actual position, on a sandbar about a kilometre outside the Zeebrugge harbour wall, meant that she came to rest half-submerged with her starboard side still above the waterline. 193 of the 539 aboard died in the first ten minutes, most from cold shock in the water or from being trapped in compartments that flooded as she rolled. 346 survived, many clinging to the starboard side of the hull and rescued by Belgian tugs and helicopters through the next hour.

The Vessel The MV Doña Paz was a Japanese-built passenger ferry, 2,215 gross tons, 93 metres long, originally launched in 1963 as the Himeyuri Maru for the Japanese domestic ferry trade between Kagoshima and Okinawa. She was sold in 1975 to the Philippine interisland operator Sulpicio Lines, renamed Don Sulpicio, and worked the Manila-Cebu route until a fire damaged her in a port incident in 1979. She was rebuilt and returned to service in 1981 under the name Doña Paz. Her certified passenger capacity after the 1981 rebuild was 1,518, a figure agreed with the Philippine Coast Guard and published on her bridge ticket. Her actual carrying capacity at any given voyage was larger, because Philippine interisland practice allowed tickets to be sold on deck and in passageway space, and because infant passengers did not count toward the manifest limit. She was typically operated at between 1,800 and 2,500 aboard, depending on the season. The Sulpicio Lines fleet of which she was the flagship had a long record of regulatory violations, fires, and strandings; the Philippine Coast Guard had cited the line for manning, lifeboat, and certification failures in every year since 1979. None of these citations had resulted in the cancellation of her certificate. The Philippine ferry industry in the 1980s was the most lightly regulated major passenger-carrying maritime sector in the world. The Disaster The Doña Paz burned and sank within two hours. The Vector burned longer and sank at around 02:30 on 21 December. The sea surface around both ships burned for most of the night on a sheet of flaming kerosene and gasoline, fed by the Vector's ruptured cargo tanks. The water of the Tablas Strait that night was shark-infested, as the area has long been known to be; the survivors who jumped from the ferry's rails into water that was already on fire had first to clear the burning layer and then to swim in water where survivors were being taken. 24 of the Doña Paz's passengers and two of the Vector's crew were rescued by the passing Philippine ship Don Claudio at dawn on 21 December. No other survivors were recovered. 4,386 people died, by the final count accepted by the Philippine Board of Marine Inquiry; this is the figure preserved on the official marker. Outside analysts, survivor advocacy groups, and independent researchers have argued that the true toll was between 4,500 and 5,000, based on the fact that the Sulpicio Lines manifest was known to exclude at least 700 non-ticketed passengers and that no complete manifest of deck passengers was ever produced. The Doña Paz herself came to rest at 545 metres, in deep abyssal water, and has never been located.

Iraq apologised and paid indemnity; Saddam Hussein personally called the incident an accident, an explanation that has held up through every subsequent review. The Vessel USS Stark (FFG-31) was an Oliver Hazard Perry-class guided-missile frigate of the United States Navy, commissioned at the Todd Pacific Shipyards in Seattle, Washington, on 23 October 1982. She was 138 metres long, 4,100 tons, armed with a single Mark 13 missile launcher (capable of firing SM-1 Standard surface-to-air missiles and Harpoon anti-ship missiles), a single 76mm Oto Melara gun, two triple 324mm torpedo tubes, and a Phalanx 20mm close-in weapons system. Her designed speed was 29 knots on a single-shaft gas turbine arrangement. Her commissioning into the Atlantic Fleet placed her in a class that was the principal American frigate type of the 1980s and early 1990s. The specific tactical situation in May 1987 was complex: Iran and Iraq had been at war since September 1980; both sides were conducting attacks on third-party shipping in the Persian Gulf; and the Iraqi air force was using French-supplied Mirage F1 fighter-bombers armed with Exocet missiles to attack Iranian oil infrastructure and shipping. The Disaster The first of two Exocet AM-39 missiles fired by the Iraqi Mirage F1 reached USS Stark at 21:09 on 17 May 1987. The missile struck Stark's port side at Frame 43, near her crew berthing compartments. The warhead failed to detonate on impact but the missile's residual rocket motor fuel ignited and produced a fire that spread rapidly through the forward port-side crew compartments. Twenty-five seconds after the first missile impact, the second Exocet reached Stark at Frame 75, penetrating her port side and detonating in her Combat Information Center. The CIC detonation killed the CIC watch team and destroyed Stark's primary combat management systems. The ship was, from the moment of the second missile impact, essentially defenceless and unable to coordinate her own damage control. Stark's surviving crew fought the resulting fires for 24 hours. The platform's automatic firewater deluge system, which should have activated immediately, failed to do so: the deluge pumps had been placed on manual control during an unrelated maintenance activity earlier in the day and had not been returned to automatic. The manual restart was attempted but took time. Twenty-two minutes into the fire, at 22:20, the main gas riser from the Tartan platform ruptured. The Tartan riser carried gas at 120 bar pressure; when it breached, approximately 15 tonnes of gas released in three seconds and ignited in a fireball that rose 100 metres above the platform, twice the platform's own height. The Tartan rupture destroyed the control room, the communications centre, and the main emergency control station.
